Network Rail Certified Medical Screening, Network Rail D.A Screening

20-45 Minutes

1 Per 45 Minute Appointment

Must be a minimum age of 16.

A profile set up on Sentinel (www.railsentinel.co.uk).

Proof of National Insurance- Must bring proof of National Insurance on an official document such as P45, P60, Pay Slip, Letter from HRMC.

Photo ID – For British Nationals – a current and valid recognised, branded form of identity token, used for accessing the RMI (Railway Maintained Infrastructure) such as a Sentinel Card, full driving licence, passport, biometric ID Card, resident permit as per Home Office guidance or ‘ValIDate’ card. For Non-British Nationals – a current and valid passport is required.

You will be asked to complete a Medical Deceleration from which provides information about your medical past history. Please arrive 15 minutes before your appointment to ensure all paperwork is filled out ready for your appointment time. Failing to do this could result in you losing your appointment time.
